    A semantic metadata enrichment software ecosystem (SMESE) : its prototypes for digital libraries, metadata enrichments and assisted literature reviews

    Contribution 1: Initial design of a semantic metadata enrichment ecosystem (SMESE) for Digital Libraries The Semantic Metadata Enrichments Software Ecosystem (SMESE V1) for Digital Libraries (DLs) proposed in this paper implements a Software Product Line Engineering (SPLE) process using a metadata-based software architecture approach. It integrates a components-based ecosystem, including metadata harvesting, text and data mining and machine learning models. SMESE V1 is based on a generic model for standardizing meta-entity metadata and a mapping ontology to support the harvesting of various types of documents and their metadata from the web, databases and linked open data. SMESE V1 supports a dynamic metadata-based configuration model using multiple thesauri. The proposed model defines rules-based crosswalks that create pathways to different sources of data and metadata. Each pathway checks the metadata source structure and performs data and metadata harvesting. SMESE V1 proposes a metadata model in six categories of metadata instead of the four currently proposed in the literature for DLs; this makes it possible to describe content by defined entity, thus increasing usability. In addition, to tackle the issue of varying degrees of depth, the proposed metadata model describes the most elementary aspects of a harvested entity. A mapping ontology model has been prototyped in SMESE V1 to identify specific text segments based on thesauri in order to enrich content metadata with topics and emotions; this mapping ontology also allows interoperability between existing metadata models. Contribution 2: Metadata enrichments ecosystem based on topics and interests The second contribution extends the original SMESE V1 proposed in Contribution 1. Contribution 2 proposes a set of topic- and interest-based content semantic enrichments. The improved prototype, SMESE V3 (see following figure), uses text analysis approaches for sentiment and emotion detection and provides machine learning models to create a semantically enriched repository, thus enabling topic- and interest-based search and discovery. SMESE V3 has been designed to find short descriptions in terms of topics, sentiments and emotions. It allows efficient processing of large collections while keeping the semantic and statistical relationships that are useful for tasks such as: 1. topic detection, 2. contents classification, 3. novelty detection, 4. text summarization, 5. similarity detection. Contribution 3: Metadata-based scientific assisted literature review The third contribution proposes an assisted literature review (ALR) prototype, STELLAR V1 (Semantic Topics Ecosystem Learning-based Literature Assisted Review), based on machine learning models and a semantic metadata ecosystem. Its purpose is to identify, rank and recommend relevant papers for a literature review (LR). This third prototype can assist researchers, in an iterative process, in finding, evaluating and annotating relevant papers harvested from different sources and input into the SMESE V3 platform, available at any time. The key elements and concepts of this prototype are: 1. text and data mining, 2. machine learning models, 3. classification models, 4. researchers annotations, 5. semantically enriched metadata. STELLAR V1 helps the researcher to build a list of relevant papers according to a selection of metadata related to the subject of the ALR. The following figure presents the model, the related machine learning models and the metadata ecosystem used to assist the researcher in the task of producing an ALR on a specific topic

    The Role 3 Multinational Medical Unit at Kandahar Airfield 2005–2010

    In late 2005, Canadian Forces Health Services (CFHS) was tasked with the command of the NATO Role 3 Multinational Medical Unit (R3MMU) on Kandahar Airfield in southern Afghanistan. Preparations drew on past experience and planning. Eight complete hospital contingents were trained and deployed in rotation. Near-reality simulation training was undertaken with the combat brigade, including complete deployment of the field hospital in the exercise area. Standard operating procedures (SOP) were developed and applied by each rotation so successfully that they were adopted by the new command in late 2009. The Canadian period at R3MMU had the highest survival rate ever recorded for victims of war. Lessons learned are being applied among victims of the conflict and trauma. The experience of the R3MMU was used to successfully deploy a hospital as part of the earthquake relief effort in Haiti in 2010. The training protocols and SOP are being applied to disaster preparedness in Canadian civilian hospitals

    Multi-drug resistant Acinetobacter infections in critically injured Canadian forces soldiers

    <p>Abstract</p> <p>Background</p> <p>Military members, injured in Afghanistan or Iraq, have returned home with multi-drug resistant <it>Acinetobacter baumannii </it>infections. The source of these infections is unknown.</p> <p>Methods</p> <p>Retrospective study of all Canadian soldiers who were injured in Afghanistan and who required mechanical ventilation from January 1 2006 to September 1 2006. Patients who developed <it>A. baumannii </it>ventilator associated pneumonia (VAP) were identified. All <it>A. baumannii </it>isolates were retrieved for study patients and compared with <it>A. baumannii </it>isolates from environmental sources from the Kandahar military hospital using pulsed-field gel electrophoresis (PFGE).</p> <p>Results</p> <p>During the study period, six Canadian Forces (CF) soldiers were injured in Afghanistan, required mechanical ventilation and were repatriated to Canadian hospitals. Four of these patients developed <it>A. baumannii </it>VAP. <it>A. baumannii </it>was also isolated from one environmental source in Kandahar – a ventilator air intake filter. Patient isolates were genetically indistinguishable from each other and from the isolates cultured from the ventilator filter. These isolates were resistant to numerous classes of antimicrobials including the carbapenems.</p> <p>Conclusion</p> <p>These results suggest that the source of <it>A. baumannii </it>infection for these four patients was an environmental source in the military field hospital in Kandahar. A causal linkage, however, was not established with the ventilator. This study suggests that infection control efforts and further research should be focused on the military field hospital environment to prevent further multi-drug resistant <it>A. baumannii </it>infections in injured soldiers.</p

    Drotrecogin Alpha (Activated) in Two Patients with the Hantavirus Cardiopulmonary Syndrome

    Hantavirus cardiopulmonary syndrome (HCPS) is associated with rapid cardiopulmonary collapse from endothelial injury, resulting in massive capillary leak, shock and severe hypoxemic respiratory failure. To date, treatment remains supportive and includes mechanical ventilation, vasopressors and extracorporeal membrane oxygenation, with mortality approaching 50%. Two HCPS survivors initially given drotrecogin alpha (activated) (DAA) for presumed bacterial septic shock are described. Vasoactive medications were required for a maximum of 52 h, whereas creatinine levels and platelet counts normalized within seven to nine days. Given the similar presentations of HCPS and bacterial septic shock, empirical DAA therapy will likely be initiated before a definitive diagnosis of HCPS is made. Further observations of DAA in HCPS seem warranted

    À Montréal, au début des années 2000, une série d'événements violents mènent à d'importantes opérations policières qui, jumelées à un grand battage médiatique, propulsent à l'avant-scène les gangs de rue. Jusque-là considérés comme de la petite délinquance et essentiellement associés aux difficultés d'intégration des nouveaux immigrants, ces groupes apparaissent comme une nouvelle menace à la sécurité publique. Qu'en est-il vraiment ? Curieusement, il n'existe aucun consensus quant à la définition du phénomène, même si certaines caractéristiques - la jeunesse des membres, le caractère illégal des activités ou l'identité commune - se retrouvent dans tous les cas de figure. Comment prévenir et traiter ce phénomène ? Dans cet ouvrage, des criminologues et d'autres spécialistes chevronnés font un tour d'horizon de tous les aspects liés à cette forme de criminalité déjà bien installée dans plusieurs pays, mais dont l'existence au Québec commence à peine à être admise et comprise
